• in honor of a physician or other healthcare provider; or
• in memory of a loved one; or
• to a specific hospital, department, center, or program.

We will gladly inform the person being honored, or the family of the person being memorialized. Just be sure to provide us with the names and addresses of the persons who should be notified.
